Silver Lake Summer Hoop Basketball Camp

The required documents must be completed and sent to the following address before camp begins:

• 2026 Silver Lake Summer Hoops Basketball Camp Registration Form (PDF below can be printed)
• Personal Check/Money Order (Checks should be made payable to Coach Dick Steele)
• Silver Lake Summer Hoops Medical Waiver Form (PDF below can be printed)
• Up-to-Date Medical Examination Record (Physical Form from Primary Care Physician)

PRINTABLE REGISTRATION FORMS AND INFORMATION BELOW:

• The email address silverlakesummerhoops@yahoo.com functions as a general mailbox for addressing inquiries and sending registration confirmations when they are granted. However, it is not designated for the receipt of registration documents.

• Please note that registration is accepted **only by mail**—no exceptions. Silver Lake Summer Hoops Basketball Camp does not accept any documentation via email, fax, or online form submissions.   While we trust that the USPS will deliver your registration documents without issue, we recommend obtaining a tracking number from the post office (it does not need to be certified or require a signature for delivery-this is for your peace of mind)

• Registrations are processed on a first-come, first-served basis and must include all necessary documentation along with either a nonrefundable deposit of $175.00 or full payment per registration. Confirmation for your child(ren)'s reserved spot at camp will only be provided once these requirements are met.

• Once camp has commenced, no refunds will be issued. However, a credit for future camp dates may be granted at the discretion of the Camp Director, with decisions made on a case-by-case basis.

• Registration confirmations will be emailed to the address specified on your registration form from: silverlakesummerhoops@yahoo.com.
